Position Title: Therapist - LPC, LCSW, LMHP
Opening Date: 3/23/2018
Closing Date: Ongoing -until the position is filled.
Position Description: Candidate will implement and conduct initial assessments (MSE), create person-centered treatment plans, provide individual, family, or group psychotherapy, design and implement behavior modification programs. Other duties include collaborating with physicians and other specialists to develop and implement person-centered treatment and intervention programs with patients.
Location of Position(s): Hampton Roads area
Major Duties:
The candidate will interview adults, children, and adolescents implementing assessments and creating person-centered treatment plan. The therapist will also provide in-home services including, individual, family, and group psychotherapy, and may perform psychosocial assessments. The therapist will also be responsible for updating individual service and treatment plans, communicating with various referral agencies including DSS, DJJ, Juvenile Courts, and Schools, and assisting a team of Qualified Mental Health Professional and/or (Case Managers) not limited to collaborating with other mental health professionals.

Qualifying experience for this position includes at least five (3) years of specialized experience which is directly related to the position and has equipped the applicant with the particular knowledge and skills to successfully perform the duties of the position. Master’s Degree in Psychology, Social Work, Nursing, or Counseling and must have LPC, LCSW, LMHP or PsyD status in the state of Virginia. Also, the candidate should have experience with C&A clients, Individual Service Plans (ISPs), and have proficient knowledge of clinical terminology.

Position Title: Mental Health Skill Building Counselor
Opening Date: 3/23/2018
Closing Date: Ongoing -until the position is filled.
Position Description: Under the supervision of the Clinical/Program Director, the Mental Health Skill Building Counselor will provide direct Mental Health Skill Building services that implement a service plan, which provides structure, support, and services to the client at risk for removal from the home. These services are provided in accordance with agency policies.
Location of Position(s): Hampton Roads area
Major Duties:
The candidate will conduct the following duties:

Provides direct, Mental Health Skill Building counseling services to identified client (and their families).

Ensure that the required LMHP documents have been gathered and completed to ensure that the counselor and all involved parties can participate in the preparation of treatment plan and updates.

Assists in the planning of services.

Provides client with day to day skills/education as well as support to assist the client in meeting their day to day obligations.

Models and teaches problem-solving techniques.

Provides a variety of interventions in client’s home and community as appropriate and necessary. These interventions may be behavioral, social, financial and/or from a mental health perspective.

Provides linkage to community-based services and supports; provides advocacy and support for the client as well as their family members.

Coordinates scheduling of appointments, as needed.

Coordinates and provides transportation, as needed.

Available for 24-hour crisis intervention and family support.

Is responsible for the proper and timely documentation of all information required by the Department of Mental Health, Mental Retardation and Substance Abuse Services and P.A.CS.

Maintains regular and ongoing contact with referring agencies, as well as other agencies involved in client and (family’s case).

Actively coordinates and plans discharge and aftercare plans with referring agency.

Attends interagency meetings, as necessary.

The Mental Health Skill Building Counselor shall have a Bachelor’s or Master’s Degree in the human services field (or a pre-approved discipline as identified by Medicaid). The applicant must also meet DMAS hiring requirements as well have at least one year of clinical supervision.
